For the most part, my services are provided in person at my office in Corvallis. My address is 1220 NW Kings Blvd. A map and directions can be found on my Contact page. However, for Oregon residents who do not reside or work within driving distance of Corvallis individual psychotherapy services are available, when clinically appropriate, by secure video.
Evaluate potential client's appropriateness for services or referral.
When you call for an appointment, I'll ask you a series of questions to determine if it is appropriate to schedule an assessment or make a referral to another psychotherapist.
You (and your partner, if it is a couple seeking services) will bring a completed a self-assessment form to our first session. We will review the form and discuss why you are seeking services, your current and historical behavioral health challenges, and your goals for treatment.
We will jointly determine what your goals are for treatment and how we will know when they have been accomplished.
We will actively work on achieving the goals described in the Treatment Plan. Progress will be monitored and treatment will be modified, as needed, to achieve treatment objectives.
For clients who have completed time-limited therapy, maintenance sessions provide an opportunity, if needed, to support the gains achieved during treatment.
There is no charge for the initial phone screening. For all other services, my standard fee is $205 per hour, with a sliding scale available as described below.
For clients with no insurance who lack the financial ability to pay for services at my standard rate, the discounted charge for full payment at the time of service is $125 per hour, payable in cash or by check, credit card, or PayPal. For clients who have insurance, but I am not on their carrier's panel, I can provide clients with a receipt for services which they can then submit to their insurance companies if their health plans provide out of network benefits. Such clients must make full payment for services at the time of each service and will be solely responsible for seeking reimbursement from their health plans. I will complete any paperwork an insurance company may requires in order to make a payment determination.
I am on the Pacific Source, Moda, IHN/Samaritan, and Regence BlueCross BlueShield of Oregon insurance panels. Clients with these managed care plans will pay their co-pay, co-insurance, or deductible (if not met) at the time of each service. IF YOU HAVE ONE OF THESE PLANS, BRING YOUR INSURANCE CARD TO YOUR FIRST APPOINTMENT.
I am not a Medicare provider, including Medicare Advantage plans. The regulations governing Medicare and their reimbursement rates make being a Medicare provider too difficult. I will, however, provide services to Medicare eligible individuals who will agree to pay out of pocket for services AND will sign a contract with me, as required by Medicare. Clients who have signed this contract are free to seek reimbursement through a Medigap insurer, if they have such coverage. I will provide the letter which such insurers require of providers who have "opted out."
For further details about my fees, cancellation policy, and other important details about how I work, please review my office policies information sheet.
Please review Q & A page to better understand these services and charges.
As of July 4, 2020 I am no longer accepting new psychotherapy clients. I am retiring as psychotherapist. I will, however, continue with my consulting and teaching work.
